WAIVER OF LIABILITY AND PHOTO RELEASE
Endurant Movement - Day of Healing | June 8, 2025
By signing this form, I acknowledge and agree to the following:
1. Waiver of Liability:
I understand and acknowledge that my participation in the Community Day of Healing event on June 8 2025, organized by Endurant Movement, is voluntary. I agree to assume all risks associated with my participation, including but not limited to personal injury, illness, or property damage. I hereby release, waive, and discharge Endurant Movement, its founders, directors, officers, agents, volunteers, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained during or as a result of my participation in this event.
2. Photo & Media Release:
I grant permission to Endurant Movement to take photographs, video recordings, and audio recordings of me during the Community Day of Healing event. I authorize the use of my likeness, voice, and name in any and all media formats including, but not limited to, social media, website, promotional materials, and press releases, without compensation or further approval. I understand that these materials may be used for informational, educational, and promotional purposes in support of Endurant Movement’s mission.
3. Acknowledgement:
I have read this waiver and release of liability and fully understand its terms. I sign it freely and voluntarily without any inducement. If the participant is under 18 years of age, a parent or legal guardian must sign below.
